In partnership with your physician, our diabetes educators provide a variety of outpatient services at most CHI St. Vincent locations. A physician referral is required for individual instruction, group classes, nutrition therapy and gestational education. Check with your insurance company to see if Diabetes Education is a covered service.
Diabetes Education: Individual
Educators are available to instruct you and your family on the basics of self-management and nutrition. Individual instruction in insulin therapy is also available. Our instructors will assist you in the selection of the most appropriate monitor.
Diabetes Education: Group
If you are newly diagnosed with diabetes or have been diagnosed in the past, the classes will help you improve your diabetes control. Classes are taught by a registered nurse/ diabetes educator and dietitian. Topics include:
- What is diabetes?
- Types of contributing factors
- Blood sugar monitoring
- Treatment options
- Nutrition and meal planning
- Medications and insulin
- Exercise
- Preventing and managing high and low blood sugar
- Preventing and managing complications
- Learning to live with diabetes
- Goal setting and life style change
Nutrition Therapy
Nutrition plays an important role in your health. A dietitian will provide individualized information based on your lifestyle and treatment plan. Topics include:
- Meal planning
- Carbohydrate counting
- Reading food labels
- Dining out
- Weight management
- Cholesterol and blood pressure management
Gestational Diabetes
Our Gestational Diabetes Education program will assist you with your special diabetes management needs during pregnancy. The program consists of:
- Nutrition education including an individualized meal plan – one or more sessions with the registered dietitian
- Self-management education including blood sugar monitoring, sick day care, target blood sugars – one or more sessions with a registered nurse/diabetes educator.
Pre-Diabetes
Pre-diabetes occurs when a person’s blood sugar is higher than normal but not high enough for a diagnosis of diabetes. Call us for current program options.
